   Are you using the premium version of Jetpack? As I am following along I did not have the contact us box option and now on this unit I do not have the social media button option, and googling does not seem to offer any solutions.

       Solved the issues. The extra sidebar widget will give you both the social media icons and contact us widgets.
       Let me know if you have any more issues.

       Extra Sidebar Widgets
       The “Extra Sidebar Widgets” feature includes widgets you can add to your blog. From RSS Links to Twitter Timelines and Facebook Like Boxes to social icons, this feature makes it easy to add extra functionality to your site.

       To enable this feature, visit Jetpack → Settings → Writing in your site’s dashboard. Scroll down to the Widgets section and toggle on the Make extra widgets available for use on your site including subscription forms and Twitter streams option.

       Here is a list of all Widgets added by the Extra Sidebar Widgets feature:
